Letter of recommendation- Help!

Can someone help with some tips on what schools in Canada/US accept letters of recommendation that have been written in advance? I would like to pursue a MBA in Canada or US. I have already contacted two schools, and they have indicated that my supervisors have to go online and fill out a form. My supervisors are really busy people, I feel bad asking them to go online to fill out a bunch of forms for all of the different universities I am trying to apply to (4-5 aprox.). They have offered to give me a generic recommendation letter, signed and which indicates the basics, like how many years of experience I have in the career path, my current duties, professional skills etc. I have more than 6 years of working experience.

You are worth it. Don't sabotage your entry into excellent MBAs because you are scared to ask for extra help. Surely these people have assistants, if they are this busy, who do exactly this sort of task for them.
